TERMS & CONDITIONS

Thank you for choosing The Speech Path. Please read the following terms and conditions carefully before registering for a Summer Speech Package.

  1. All sessions included in the Summer Speech Package must be used before August 28, 2026.

  2. Sessions will be scheduled during the period of June 8, 2026, through August 27, 2026.

  3. Kelly will contact you following your registration to schedule your summer sessions.

PACKAGE USE PERIOD

  1. The Summer Speech Package does not include an evaluation. Services will specifically target goals outlined in your child's current IEP. If your child does not have a current IEP or is not a current Speech Path client, a mini-evaluation must be completed prior to initiation of therapy services. The price of a mini-evaluation is $100.

  2. A short summary of progress on speech/language goals addressed will be provided after the final session.

By purchasing the Summer Speech Package, you acknowledge that you have read, understood, and agree to these terms and conditions.

Acknowledgement

  1. Sessions canceled more than 24 hours before the scheduled appointment may be rescheduled if the clinician's schedule allows.

  2. If you fail to cancel a session and “no-show” your scheduled session, you will forfeit the cost of that session.

  3. Sessions canceled by the clinician will be rescheduled.

CANCELLATION POLICY

  1. Within one week of registration for your Summer Speech Package, required clinic forms (HIPPA, Consent to Treatment, etc.) will be sent to you via Kelly’s EHR system, Simple Practice. You will have 2 weeks from receiving these forms and agreements to either complete the forms or cancel your registration. All forms and agreements must be completed prior to your child’s first session.

  2. Once clinic forms have been signed, you will receive an invoice through Simple Practice to pay for your Summer Speech Package. Once payment is received, all sales will be considered final. If for any reason Kelly is unable to continue to provide services, a refund of any unused sessions will be issued.

Forms, PAYMENT & REFUNDS

Sessions will take place in Kelly’s office at the International Spanish Language Academy (ISLA).
